Admissions / Enrollment
Enrollment Requirements
The high schools are open entry. The following items are needed for enrollment.
Enrollment packets and forms can be printed using the links in the menu on the right side of this page.

Student Registration Form
Parent/Guardian signature is required for students under the age of 18. -
Proof of Identity and Age
A photocopy of a certified birth certificate is preferred. Other acceptable documents may include a baptismal certificate, application for a social security number, original school registration records with an affidavit, or documentation from an authorized agency with custody of the student. -
Immunization Records
A photocopy of immunization records or proof of exemption is required for attendance, but is not a condition of enrollment. Homeless students have 5 calendar days after enrollment to provide proof of immunizations. -
Academic Transcripts
Transcripts from all previously attended high schools are requested, but are not a condition of enrollment. The school will provide a transcript request form and request records from prior schools. -
Arizona Residency Documentation Form
Arizona Department of Education residency documentation is required. -
Education and Career Action Plan (ECAP)

Withdrawal Form
The school will request a withdrawal form from the most recent high school attended. This is not a condition of enrollment, but is required for attendance.
Student Services and Resources
The high schools provide special education services for students with individualized education plans (IEPs). Each student with a disability receives services as determined by their individualized education plan.
